Going fully independent from scratch typically takes 6–18 months to reach a sustainable caseload, depending on your market, payer credentialing timelines, and referral network strength. Therapists who join an established platform with existing payer contracts and referral pipelines often reach a meaningful caseload in 4–8 weeks. The difference is whether you're building infrastructure or plugging into it.

What does building your own pediatric therapy practice actually involve?

At minimum: credentialing with payers (3–6 months), establishing a business entity and malpractice coverage, building referral relationships with pediatricians and schools, setting up billing and scheduling systems, and marketing yourself locally.
